A machine can have an emergency-stop button, a safety relay, and a guard switch-and still fail to reach a safe state when danger occurs.

Protective devices are often treated as individual components. In reality, machine safety depends on the complete chain: detection, wiring, logic, output switching, energy control, feedback, reset, and restart prevention. A green LED or healthy PLC bit does not prove that a contactor opened, motion stopped, pressure was removed, or restart is impossible.
