There is a particular moment in a session when you cannot name what is in front of you.
The client is describing something, and it is not quite grief, not quite shame, not quite the thing your training predicted. What you say next will either open it or close it.
This is a reference book for that moment.
The Clinical Anatomy of Emotion sets out 128 emotional states, each under the same twelve headings. What it is. What it is doing. How it shows in the body. How clients actually say it. What it may be covering, and what may be arriving disguised as it. How to tell it apart from the states it gets confused with, and the question that discriminates. What to do next. What most practitioners do instead, and why that move so often confirms the state it was meant to interrupt. What it pulls from you. Where the risk sits, and where it does not.
Two arguments run underneath it. The first is that clients are right about their situations more often than a counsellor's training predisposes them to assume, and that treating an accurate perception as a symptom is its own clinical error. The second is that the instinct to relieve discomfort is the main mechanism by which clinical material gets lost. Almost every common error described in this book is a kind one.
The states are organised in layers: the defensive and shielding states, the self-conscious and shame states, the core and primary responses, the inhibitory and depressive states, the integrated and healing states, and a lexicon of positive states that clinical writing usually skips over. Behind them sit nine appendices, including a somatic index that runs backwards from what you observed to what it might be, a differential guide to the pairs most often confused, a counter-transference check that reads your own reactions as data, a risk and escalation card, language and scripts, guidance on working with alexithymia and interoceptive difference, and client-facing handouts.
It is built to be opened rather than read. Every entry is numbered and cross-referenced, so a state points you at its neighbours and at whatever it is commonly mistaken for.
Written for qualified and trainee counsellors, psychotherapists, supervisors and allied practitioners. The framework draws on psychodynamic practice and affective neuroscience, and it says plainly where the evidence is solid, where it is contested, and where an entry is offering a considered next move rather than something with a trial behind it.
