El libro encuentra su caso de estudio más perturbador en el SÃndrome de Riesgo de Psicosis, la controvertida categorÃa introducida en la Sección III del DSM-5 para identificar personas que podrÃan desarrollar una psicosis. Gómez Pérez argumenta que esta categorÃa -definida por criterios intrÃnsecamente ambiguos y con tasas de conversión que no superan el 36% a tres años- es especialmente vulnerable a las distorsiones que la traumatización vicaria produce en el juicio diagnóstico: el umbral de alarma desplazado, la intolerancia a la incertidumbre, el sesgo de disponibilidad alimentado por memorias emocionales de crisis pasadas. La conclusión es tan incómoda como necesaria: un sistema diagnóstico que ignora el estado interno del evaluador no es más objetivo; es simplemente más ciego.
Traumatización Vicaria está dirigido a psiquiatras, psicólogos, trabajadores sociales clÃnicos, supervisores y responsables de servicios de salud mental que trabajan con poblaciones traumatizadas, especialmente en el contexto latinoamericano. No es un manual de autocuidado ni un alegato contra el diagnóstico: es una invitación a construir una psiquiatrÃa epistémicamente honesta, que reconozca al observador como parte constitutiva del acto clÃnico. Porque el terapeuta que se cuida no cuida menos a sus pacientes. Los cuida mejor, por más tiempo y con mayor precisión.
