Most organisations say they have accountability.
Many do not.
What they usually have is a mixture of policies, committees, reporting lines, escalation paths, and job titles that look sensible until something goes wrong. Then the gaps become obvious. People can explain their part of the process, but no one can clearly say who owned the decision, who had the authority to act, who was responsible for delivery, and who should have challenged the outcome before it failed.
That is not an execution problem. It is an accountability problem.
Accountability by Design is a practical guide to building accountability structures that work in real organisations, under real pressure. It is written for leaders, managers, security professionals, risk officers, compliance teams, governance practitioners, and anyone else responsible for making sure important work is owned, delivered, tested, and improved.
This book explains how to:
- Define the difference between accountability, responsibility, and oversight.
- Assign accountability at the right level of leadership.
- Align authority with ownership so people are not made accountable for outcomes they cannot control.
- Use frameworks, policies, standards, procedures, and operational models to make accountability enforceable.
- Build escalation paths that move risk and issues to the right decision-makers.
- Use assurance, metrics, governance reviews, and automation to test whether accountability is actually working.
The examples in this book are anonymised, but the lessons come from lived experience across military operations, cybersecurity, financial services, technology, risk management, and highly regulated environments.
This is not a book about adding bureaucracy. It is a book about removing ambiguity.
