The number every home care conference repeats is already out of date. Here's what's actually coming.
For over twenty years, Julio Briones has sat across the table from home care agency owners making decisions about staffing, pricing, and growth. Again and again, he heard the same statistic: Ten thousand people turn 65 every day. It's on conference slides. It's in trade press. It's treated as settled fact.
It's also wrong for the world we're living in now.
The Age Cliff traces that number back to its source, shows exactly when it stopped applying, and replaces it with the data that actually matters for anyone running a home care business, planning care for a parent, or shaping the policy that governs both: the population turning 65 is starting to shrink, while the population turning 80 and 85 is about to grow faster than the workforce or the family caregivers around it can keep up with.
This book follows that shift through every part of the system it touches:
- The demographics, corrected and sourced, showing exactly where the real pressure is concentrated and when it hits
- The family caregiver, 63 million people providing unpaid labor worth more than a trillion dollars a year, with the financial and health toll laid out in specific, documented numbers
- The direct care workforce, why the job pays what it pays, why turnover runs as high as it does, and why raising Medicaid rates doesn't automatically raise wages
- The senior, what three different households can actually afford in paid care at current rates, and what happens when the money runs out
- Private pay economics, including where every dollar of a $35-an-hour billing rate actually goes, and what happens to an agency's margin when wages rise and rates don't
- Medicaid, the six-state comparison showing how differently states ration a program that's optional everywhere and gets cut first when budgets tighten
- What's already been tried, eight documented programs and demonstrations, including the honest failures, not just the wins
- Policy on the table, twenty-two proposals scored against each other so you can see which ones actually move the needle and which ones just sound good at a podium
Every figure in this book is traced to a primary source: Census Bureau data, PHI, HRSA, the Federal Reserve, MedPAC, and congressional testimony from people who lived the numbers being discussed. Where the evidence is incomplete or the data doesn't exist yet, the book says so directly instead of filling the gap with something that sounds more finished than it is.
This isn't a book that ends with a reassurance that the system will sort itself out. The evidence doesn't support one. What it offers instead is a clear, documented picture of where the pressure is landing first, who feels it, and which responses have enough evidence behind them to actually be worth building a plan around.
Written for home care agency owners and operators making staffing and pricing decisions, policymakers weighing where scarce dollars should go, and families trying to understand what's coming before they're in the middle of it.
Julio Briones has spent more than two decades in the home care industry, from his early years as a Medicaid recruiter through operations, franchise consulting, agency ownership, and a decade running Briones Consulting Group. He wrote this book after watching his own family navigate a home care experience that fell short of what it promised, and after realizing how many of the numbers driving industry decisions had stopped being true years before anyone corrected them.
