Mental health in prisons is a major issue. The prevalence of psychiatric disorders is extremely high in prisons. However, the prevalence and patterns of psychiatric comorbidities in prisons have never been studied. The aim of this study is to describe the patterns of psychiatric comorbidities in French prisons using the log-linear model. The study cohort comprises 800 male French prisoners. 36 per cent of them suffer from at least one psychiatric disorder. Among them, over 95 per cent have multiple psychiatric conditions. The core of interactions centres on depression, in association with anxiety disorders and substance dependence. Two independent systems coexist alongside this. The prevalence of psychiatric comorbidities is extremely high amongst the French prison population. These disorders are organised into three systems: the first centred on depression and anxiety disorders, the second characterised by psychotic disorders, and the third centred on bipolar disorder. It is therefore important to develop care programmes for these specific psychiatric disorders
