The interventions that decide whether a casualty survives are performed by whoever is nearest, in the first minutes, without equipment, under threat. Field medicine is not hospital medicine performed outdoors. The clinician is not safe, the equipment is limited, the casualty cannot be moved when it would be convenient, and the interval to definitive treatment is measured in hours rather than minutes. A provider who applies resuscitation-room reasoning inside those constraints will make decisions that are correct in a hospital and wrong in a doorway.
This reference explains what changes in that environment, why it changes, and what to actually do. It works through the sequence in the order it is performed, from control of catastrophic bleeding through the airway, the chest, shock and resuscitation, head and spinal injury, limb and crush injury, burns and blast, and penetrating and blunt torso injury. It then takes up what most field references leave out: what happens when there is more than one casualty, when the casualty has to be carried, and when evacuation does not come at all.
