Cysts of the spleen are rare lesions with an incidence of 0.07%. The clinical presentation of these lesions is attributed to their potential for complications such as rupture, infection, or bleeding. Furthermore, they pose a challenge in terms of differential diagnosis with neoplastic and cystic lesions (of non-splenic origin) in the left hypochondrium. Preoperative etiological and morphological diagnosis of splenic cysts is of paramount importance because treatment depends on it. However, it can be difficult to distinguish between the different types of splenic cysts preoperatively, and diagnostic certainty is achieved only through histopathological examination of the specimen. The therapeutic management of these cystic lesions of the spleen has evolved significantly over the past three decades with the advent of spleen-preserving techniques designed to avoid the infectious complications of total splenectomy, particularly fulminant sepsis. However, the choice between radical and conservative surgery remains difficult given their respective complications and the diversity of clinical situations.
