A breast needle core biopsy is a strange kind of specimen. It is deliberately incomplete. It samples a lesion the radiologist targeted, on purpose does not remove it, and then asks the pathologist to commit, on two or three cores, to a diagnosis that will decide whether a patient is reassured, sent to surgery, or started on systemic therapy.
Atlas of Breast Core Biopsy Diagnosis is built for that specific task. Not for confirming a diagnosis you already have, though it does that too, but for the harder cognitive work that comes first: seeing a pattern at low power, narrowing a differential against limited material, deciding what the core can and cannot establish, and turning that judgment into language a clinician can use.
It opens from two directions.
- By pattern, through a dedicated low-power navigation atlas that takes you from what you see to where it is discussed. This is the path for the unknown core.
- By entity, in the conventional way, when you already suspect a diagnosis. Each entity carries a pattern tag pointing you to the neighbors that share its morphology.
Four boxes recur in every entity chapter, and they are what distinguish this atlas from a catalog:
- Limited-Tissue Verdict, stating plainly what the core establishes, what it does not, and the recommended next step
- Sign-Out Language, giving illustrative wording in three registers: clean, hedged, and recommendation
- Radiologic Concordance Check, separating the result that permits sign-out from the discordant one that mandates excision or rebiopsy regardless of benign morphology
- Pitfalls, naming the overcall and undercall traps specific to that entity
What the book covers:
- Foundations: the limited-tissue mindset, specimen handling, radiologic-pathologic concordance as a working method, the pattern atlas, and a pattern-based ancillary testing toolkit
- Normal breast, inflammatory and reactive lesions, and specific infections
- Adenosis and sclerosing lesions, benign papillary tumors, adenomyoepithelial neoplasms, and fibroepithelial tumors
- Epithelial proliferations: UDH, ADH, FEA, DCIS, and lobular neoplasia including ALH and LCIS
- Invasive disease: no special type, lobular, tubular and cribriform, mucinous and papillary, medullary pattern, metaplastic, apocrine and salivary-type, and the rare special types
- Mesenchymal lesions, lymphoid and hematopoietic tumors, and metastases from nonmammary primaries
- The treated breast, post-needling change, and lesions in men and children
- A closing chapter gathering the diagnostic pitfalls and mimics of malignancy, plus IHC reference tables
Biomarker interpretation follows current CAP and ASCO guidance, with attention to low-level HER2 expression in the metastatic setting. Where practice is evolving or expert opinion diverges, the text says so instead of presenting one view as settled.
Robert B. Childers has written a reference meant to be open beside the microscope, not read once and shelved. Its purpose is stated plainly in its own pages: extract the maximum defensible conclusion from the tissue you have, recognize when that conclusion is provisional, and communicate both the diagnosis and its limits clearly. When the core and the imaging disagree, the most valuable skill is knowing to say so.
