Jake's empire thrives, but when his best friend and business partner, Nate, becomes a Tranqer, the true horror of his actions hits home.
Guilt-ridden and desperate, Jake decides to find a way to reverse the effects of the drug. He teams up with Mia, a former addict who claims to know a scientist working on a cure. Their search takes them through a city in ruins, with gangs, looters, and Tranqers lurking at every turn. As they close in on the scientist's last known location, Jake realizes Mia has her own agenda: she wants revenge on the drug lords, including Jake, who profit from others' suffering. Betrayed and wounded, Jake presses on alone, even as the Tranqers close in and his chances grow slimmer by the hour.
When he finally reaches the scientist, Dr. Collins, Jake learns the cure is still in its early stages and mass production is impossible without more resources. With Tranqers closing in, Jake and Collins make a desperate escape, hoping to regroup and find a way to produce the cure. Meanwhile, Mia, driven by vengeance, rallies other survivors to attack the dealers, igniting a war in the streets. Jake must decide whether to flee with the cure for his own survival or return to help those he has wronged.
Haunted by Nate's fate and the destruction he helped unleash, Jake chooses to fight. He returns to the city, joining forces with Mia and the survivors to take down the remaining drug lords and secure Collins' lab for mass-producing the cure. In a final showdown, Jake faces the consequences of his past as Tranqers and dealers clash in a brutal battle. With Mia's help, he manages to distribute the cure, but not before being mortally wounded by a Tranqer.
As the city slowly recovers, Mia takes up the mantle of distributing the cure, determined to rebuild and make amends for her own past. The Tranq epidemic begins to recede, leaving Boston scarred but hopeful. Jake's sacrifice becomes a rallying cry for the survivors, a symbol of redemption and the cost of ambition.
