Background. Ridge preservation is a goal many dental practitioners find desirable after the loss of a patient's tooth. Acellular dermal matrix (ADM) has been used in ridge preservation, however it has yet to be determined whether thickness of the material makes a difference in regenerative procedures. Methods. Seventeen sites in fourteen patients were evaluated in a randomized clinical trial. Group A (ADM GBR) had 8 sites and group B (ADM) had 9 sites. Phase I consisted of tooth extraction and ridge preservation with autogenous bone using ADM as a membrane. Phase II surgery consisted of implant placement. A core biopsy was obtained from the osteotomy site. Soft tissue and bone measurements were made. A students t-test was used to analyze the results. Results. Connective tissue and epithelium thickness gained a mean of 2.5 mm from the facial aspect for group A and a mean gain of 1.83 mm of soft tissue thickness for group B. Bucco-lingual dimension resulted in a mean loss of 1.25 mm for group A and a mean loss of 0.33 mm for group B. Apico-occlusal dimension resulted in a mean loss of 0.88 mm for group A and a mean loss of 0.72 mm for group B. Differences between groups were not statistically significant for any measured parameters (p = 0.05). Conclusions. Acellular dermal matrix thickness of 0.5--1.6 mm used as a membrane with autogenous bone in ridge preservation reduced alveolar ridge resorption after tooth extraction to allow dental implant placement without further grafting.
