Am 12. März 1995 reiste Mazari nach Chahar Asiab, einer Einladung der Taliban zu Friedensgesprächen folgend. Er wurde festgehalten. Am folgenden Tag - 13. März 1995 - wurde er getötet. Sein Leichnam, der Spuren von Folter aufwies, wurde später in der Provinz Ghazni gefunden.
