L'intrigue commence par un mensonge audacieux. Planchet, le chef respecté et craint de la Cour des Miracles, désespère de ne pas avoir de fils pour lui succéder sur le trône souterrain de Paris. À la naissance de sa fille Jean, il décide de dissimuler son véritable sexe à la communauté et de l'élever rigoureusement comme un homme. Jean grandit ainsi dans les égouts et les ruelles sombres, développant une force physique impressionnante et une maîtrise exceptionnelle de l'épée. Pour parfaire son éducation, son père l'envoie étudier auprès du docteur Girardán, un médecin profondément charitable et médium spirituel qui guérit les pauvres et garde jalousement son secret.
Parallèlement, nous suivons le destin de Jeanpaul, un jeune noble accompli et escrimeur d'élite. Il est le fils de Paulette, une ancienne mendiante de la Cour des Miracles, et du noble Jean de Luzardo. Le couple a été réuni grâce au sacrifice de Suzanne, la première épouse de Luzardo, devenue paraplégique pour racheter une faute karmique d'une vie antérieure, et qui a orchestré leur bonheur avant de mourir en paix. Lorsque Jean et Jeanpaul se rencontrent chez le docteur Girardán, une étincelle spirituelle immédiate jaillit entre eux, souvenir inconscient de leurs nombreuses incarnations passées comme compagnons d'âme.
Mais la tragédie guette. Safeth le Turc, un bandit colossal et cruel, profite de la maladie de Planchet pour usurper le titre de roi des mendiants. Rempli de haine envers Jean, Safeth planifie de cambrioler la demeure du duc de Luzardo pour se venger. Apprenant le complot, Jean décide de défendre seule, vêtue de noir et masquée, la maison de l'homme qu'elle aime. Au cours de l'affrontement nocturne, Jean est grièvement blessée à la tête par une balle. Sauvée de justesse par l'assistant du docteur, le géant Lenoir, son secret est enfin mis au jour: pour lui sauver la vie, le docteur Girardán réalise une transfusion sanguine directe depuis le bras de Jeanpaul. En découvrant la vérité sur sa féminité, Jeanpaul et sa famille l'accueillent avec un amour inconditionnel.
Alors que l'horizon semble s'éclaircir pour les deux amants, l'Inquisition frappe cruellement en arrêtant le docteur Girardán sous l'accusation de sorcellerie et en le condamnant au bûcher. Malgré la perte de ce guide spirituel, son héritage d'amour et de charité demeure vivant. Enrichie par la fortune secrète que son père lui a léguée en Suisse, Jean décide d'aider ses nombreuses soeurs et de dissoudre la Cour des Miracles en offrant des terres aux mendiants pour rebâtir leur vie. Le Roi des Mendiants est une démonstration vibrante que les liens de l'esprit sont éternels et que seul l'amour a le pouvoir de transformer la douleur en une lumière immortelle.
